Managed by St Helens GPs, this unique and innovative service delivers primary care on a home visiting basis to the most vulnerable in our community.

Registered patients benefit from proactive care that includes regular reviews & health needs assessment as well as the more urgent care services provided by traditional GPs.

Our medical services are multi disciplinary and integrated for each patient’s individual needs. We work in partnership with individuals and organisations committed to care of our elderly population in the community.

We use the Royal College of General Practitioners bench marks of excellence as our daily working standards

Repeat prescriptions can be ordered through your standard pharmacy service or via fax, post with SAE or online
via our website:

Please allow 48 hrs for procesing of prescriptions. We can arrange the ordering, collection and delivery of your repeat prescription via a local pharmacy of your choice.
